PCA Direct Care Provider 

JOB DESCRIPTION



Function or purpose of the position:

To provide training for persons using our services that will allow them to lead Independent lives both at home and in the community.



Duties that are essential job functions or tasks to the specified position:

  • Train individuals to attend to their own needs, while helping them to become independent.

  • Be familiar with and adhere to agency policies and procedures.

  • Assure completion of all IP assessments.

  • Assist in general care, upkeep, and maintenance of individuals living area.

  • Cooperate with supervisor and co-workers on any assignment or programs and administering needed medication and first aid.

  • Provide the opportunity for community involvement in the person’s lives.

  • Possess knowledge of emergency procedures and be responsible for each consumer’s safety and welfare when on duty.



Additional duties of importance required of the position:

  • Maintain appropriate communication with management and other staff as needed.

  • Assist with the medical and dental concerns of the consumers.

  • Attend and participate in workshop conferences or other in-services as required.

  • Complete all weekly and monthly paperwork in a proper timely fashion.

  • Ensure compliance with DDS Rules and Regulations and other appropriate regulatory mandates.

  • Perform other duties as assigned.



Other knowledge, skills, abilities that are necessary when performing the position:

  • Must pass background and motor vehicle checks.

  • Human services experience is desirable.

  • Needs to possess the ability to communicate effectively in order to work productively in a team oriented environment.

  • Must have High School Diploma or equivalent.

  • Must posses valid driver's license.

  • Must be at least 18 years of age.



Physical demands:

While performing the duties of this job, the employee is regularly required to sit; use hand to finger, handle, or feel objects, tools, or controls; reach with hands and arms; hear and have the ability to communicate on a verbal basis. The employee must have adequate visual acuity with or without correction and is required to lift heavy weights up to 50 lbs. and help with transporting and assisting consumers, to include lifting them when they need help. The employee is required to travel to meetings, training, Agency facilities and other job related locations.
