Day Program Direct Care Provider

JOB DESCRIPTION



Function or purpose of the position:

  • To integrate into the community and create positions of employment for our consumers.

  • To provide care and supervision to developmentally disabled adults in a vocational setting.


Duties that are essential job functions or tasks to the specified position:

  • Assist in all aspects of vocational care, programming, and personal development activities.

  • Assist persons using our services to become as independent as possible and a socially accepted member of our community to the best of the person’s ability.

  • Assist in the organization of Supported Employment (SE) related activities for persons using our services. (To include accompanying consumers if necessary.)

  • Assist in general care, upkeep, and maintenance of facility and grounds. Keep supervisor informed of any maintenance needs.

  • Provide direct care services to all persons using our services.

  • Develop and coordinate community Supported Employment (SE) activities.

  • Provide on-site training, monitoring, and follow-up services with employment trainees which may include night or weekend hours.

  • Accurately record SE work hours and earnings, complete SE attendance sheets, and submit both to supervisor on a monthly basis.


Additional duties of importance required of the position:

  • Maintain appropriate communication with management and other staff as needed.

  • Assist with the medical and dental concerns of the consumers.

  • Attend and participate in workshop conferences or other in-services as required.

  • Complete all weekly and monthly paperwork in a proper timely fashion.

  • Ensure compliance with DDS Rules and Regulations and other appropriate regulatory mandates.

  • Perform other duties as assigned.


Other knowledge, skills, abilities that are necessary when performing the position:

  • Must pass background and motor vehicle checks.

  • Human services experience is desirable.

  • Needs to possess the ability to communicate effectively in order to work productively in a team oriented environment.

  • Must have High School Diploma or equivalent.

  • Must posses valid driver's license.

  • Must be at least 18 years of age.


Physical demands:

While performing the duties of this job, the employee is regularly required to sit; use hand to finger, handle, or feel objects, tools, or controls; reach with hands and arms; hear and have the ability to communicate on a verbal basis. The employee must have adequate visual acuity with or without correction and is required to lift heavy weights up to 50 lbs. and help with transporting and assisting consumers, to include lifting them when they need help. The employee is required to travel to meetings, training, Agency facilities and other job related locations.
